Watch the LEDs. If an input's red LED follows the field contact but the controllers never see the change, the fault is more likely in the backplane cabling or the EMIO path than in the ECTB's input circuit.

Dead relay output? The coil drivers live on the EMIO side and receive 24 V dc from the controllers. Confirm the command is being issued and the D-sub connections are seated before assuming a welded or failed relay on the ECTB.

Check the customer side. Form-C terminals switch externally supplied power. A missing external 125 V dc feed looks exactly like a dead output relay from the operator's chair.
Genuine ECTB failures do happen — relay contact wear on frequently cycled outputs, damaged terminal blocks, connector faults — and in those cases swapping the board is the fix.
